My bet is that on the prior 12-psi dyno runs you cracked a ring land (or worse), and it limped along until you loaded it again. But who knows, definitely failed piston though.

Failed ring lands are common in stock pistons when they get heated up with FI. Probably could have made them last longer if you would have ran it a bit on the richer side (using fuel to lower combustion temps) or had an alcohol injection system, but you still would have been on the hairy edge with a stock short block.
